Almost everything sold as a research peptide rests on animal work. Tesamorelin is one of the few compounds in this class with substantial randomised human data, which makes it useful for showing how such data should be read.
A 2026 systematic review pooled four randomised controlled trials covering 909 people with HIV-associated lipodystrophy [1]. Pooled numbers are worth more than any single trial, and they let us put figures on a page rather than adjectives.
Tesamorelin is growth hormone releasing hormone with a modification at the N-terminus. Natural GHRH is cleared within minutes by DPP-4; the modification blocks that cleavage. The compound does not supply growth hormone — it asks the pituitary for it, which means somatostatin and IGF-1 feedback remain in the loop. That is the whole design, and it is why the literature on it reads differently from the literature on synthetic growth hormone.
Against placebo, the pooled analysis reported reductions in visceral adipose tissue and waist circumference, changes in trunk fat and lean mass, and small shifts in lipid ratios — a compartment-specific pattern of interest to growth-hormone physiology. Effect sizes are in the source [1].
These are modest effects in one specific clinical population and should not be generalised beyond it.
The same review reported higher discontinuation rates in the tesamorelin arms and adverse effects consistent with growth hormone elevation. Effects — both wanted and unwanted — were more pronounced in the trials running 26 weeks or longer. Glucose control was not meaningfully disturbed, which had been a specific concern given what growth hormone does to insulin sensitivity.
A compound where both the measured effects and the discontinuation rate grow with duration is not a simple story, and the honest summary is that the trade-off is real rather than negligible.
Every participant in that pooled analysis was living with HIV and on antiretroviral therapy. Lipodystrophy in that context is a particular metabolic problem with a particular cause, and nothing in the data speaks to anyone outside it. The trials also ran to 52 weeks at most — what happens over years, or after stopping, is not in the record.
Tesamorelin received regulatory approval in 2010 for that indication alone. Approval is a statement about one use in one population, not a general endorsement, and it is routinely quoted as though it were the latter.
Lyophilized, reconstituted with bacteriostatic water, refrigerated once in solution and kept dark. Like the other GHRH analogs it will not forgive being shaken.
Supplied for laboratory and research use only — not for human or veterinary use. The above is a summary of published trial data, and nothing in it describes how a person should use anything.
